The Visiting Nurse Service of Northeastern New York provides Home Healthcare Services provided by professionals such as a Registered Nurse (RN), Licensed Practical Nurse (LPN), Home Health Aide (HHA), Physical Therapist (PT), Occupational Therapist (OT), Speech Therapist (ST), Medical Social Worker (MSW), Registered Dietician (RD), Respiratory Therapist (RT) and Spiritual Services provided by a Chaplain to help you manage your care in the comfort of your own home. Whether you are returning home from the hospital, or have a skilled medical need and striving to live independently in your home, we can help you. Our Care Choices Palliative Care service is available for patients to help them with their complex skilled medical needs.

Care Central Health Homes
The VNS is proud to be taking the lead to bring one of New York State’s first "health homes" to our community. This new model of care will change the way healthcare is delivered, making it better, more accessible, efficient and cost-effective. New York State Health Home Eligibility involves Medicaid persons with Two (2) Chronic Conditions; or One (1) Single Qualifying Condition (HIV/AIDS or Serious Mental Illness or SMI). Services include: Comprehensive Care Management, Health Promotion; Transitional Care including appropriate follow-up from inpatient to other settings, Patient and Family Support, Referral to Community and Social Support Services, use of Health Information Technology to link services.
