Care Central is one of New York State’s first “health homes,” an innovative model of care that provides personalized clinical and community support and service coordination aimed at improving the health and well-being of the community.
The Visiting Nurse Service of Schenectady and Saratoga Counties (VNS) is leading this unprecedented effort with the help of numerous partners, including Ellis Medicine, Hometown Health, health insurers and community organizations.

Care Central gives qualified patients a single contact for all healthcare and community service needs. Care Central’s health services “Navigators” help to link patients to doctors and serve as a support team whenever questions, guidance or assistance in managing care are needed. We make it easy to get better healthcare by coordinating treatment across all providers – whether at home, an outpatient office or in the hospital.
Find the Care You Need
Care Central is the place participants can turn if they:
are not feeling well;
need a doctor;
are uninsured or underinsured;
have health questions;
need mental health services;
need help with medications;
need referrals to specialty services (e.g., dental care, diabetes management, nutritional support);
have social service needs (e.g., transportation, housing, food).
It’s Fast, Easy and Free – 24 hours, 7 days a week
Health Navigators are there to help with healthcare, housing and other services. Our Care Central Navigators visit participants at home or in the hospital if necessary.
Our Navigators will answer questions, link you to care providers and coordinate services. Care Central Navigators help you get good care even if you can’t pay.
Who is Eligible?
Currently, Care Central is reaching out to an identified group of people who live in Schenectady and Saratoga Counties who don’t have a doctor, suffer from chronic conditions, and/or have mental health concerns.
Soon, Care Central will expand and be available to everyone in our coverage area with uncoordinated or high healthcare, specialty service, social service and/or mental health service needs.
