Currently we are especially seeking qualified candidates for the following positions:

Title: Clinical Nurse Liaison

Status: Full Time
Hours: Monday-Friday, 8am-4:30pm.

Job Summary:

The Clinical Nurse Liaison is responsible for growing key service line areas as defined by management. As a critical member of the clinical sales and marketing team, the Clinical Nurse Liaison works closely with hospital discharge planners, social workers, case managers, physicians, management, inpatient unit managers and education staff to generate referrals. Develops and maintains new and current relationships with assigned hospitals, physicians, skilled nursing facility and home health agencies. The Clinical Nurse Liaison also provides initial and on-going assessments of patients and manages patients admitted to VNS.

Primary Functions:

The main duties of a Clinical Nurse Liaison include coordinating communications between physicians, discharge planners, case managers, medical staff and caregivers regarding the admission and discharge of patients VNS. The Clinical Nurse Liaison assesses referrals and makes sure they meet the facility's admission criteria. In addition, Clinical Nurse Liaison must establish and develop relationships within the hospital and larger community by participating in events, scheduling speaking engagements and working closely with other members of the clinical team.

  • Assesses potential patient's condition and past medical history and obtains appropriate documentation to support admission using our admission process.
  • Educate/sell patients, families, referral sources on our programs, services to secure admission.
  • Coordinates the completion of the admission process and ensures that appropriate resources are obtained for a successful transfer to facility or home care. Provides the facility or receiving team with appropriate information to facilitate continuity of care.
  • Collaborates with the attending physician(s) to effectively manage the patient and ensure that transfers to other care sites are expedited as soon as possible.
  • Serves as consultant to other professionals including the hospital staff by providing expertise. Consults and collaborates with physicians as appropriate. Ensures clients meet criteria for homecare.
  • Adheres to compliance with all regulatory (COP’s, CMS, NYSDOH, etc.,) and agency policies and procedures in addition to standards of care, quality and productivity.
  • Comply with HIPAA and Data Privacy Practices.


  • A minimum of one to two years working in the health care industry
  • RN licensure required, BSN degree or a clinical degree is a plus


Title: Registered Nurse - CHHA

Status: Full Time
Hours: 8am-4:30pm. Shifts may vary based on visit schedule and patient needs.

Primary Functions:.

  • Provide a full range of direct care treatment and services to clients and families in their homes under the order of a physician
  • Provide integrated client care utilizing an interdisciplinary approach to disease prevention and health promotion
  • Educate client’s and their family in self-care so that optimal health may be achieved
  • Utilize community resources to promote the health of the client, family and community


  • Registered Professional Nurse with current New York State License
  • BLS Certification required
  • Minimum of 2 years home care experience
  • Strong knowledge base of nursing standards and home care practice
  • Strong communication and organization skills
  • Valid New York State driver’s license and insurance
  • HomeCare HomeBase and Point Care experience preferred
  • BSN preferred

Productivity Expectations:

  • 5 Visits per day on average
  • Caseload of 25-30 patients
  • Proven ability to use the agency's EMR program
