Advance Care Planning
If you experienced a sudden illness that prevented you from making your own medical decisions, who would speak for you? Would your loved ones know enough about your wishes to make decisions on your behalf? How would you ensure that you receive the kind of care you wanted? Have you had a conversation about what matters most to you?
VNS Care Choices staff is comprised of nationally certified, trained professionals who can assist you in the Advance Care Planning process. This is a process of coming to understand, reflect on, discuss and plan for a time when you cannot make your own medical decisions and are unlikely to recover from your injury or illness.
Effective planning is the best way to make sure your views are respected by your loved ones and health providers. This process will also provide great comfort to those who may make end-of-life decisions for you. Good advance care planning improves the quality of your advance directive.
With one of our Advance Care Planning facilitators, you will:
Gain understanding of Advance Directives, Health Care Proxy, Medical Orders for Life Sustaining Treatment (MOLST) and their importance
Identify your personal values and goals for living well, essentials for a good quality of life
Prepare appropriate documents that express your values and goals
Learn how to have a conversation with your loved ones about your wishes
For our patients, you will receive Advance Care Planning services as a component of our interdisciplinary and comprehensive plan of care.
For community groups, we offer a Speaker’s Bureau where we can provide presentations to your members.
For medical professionals, we offer Advance Care Planning Facilitator Certification training so you can expand services in your practices.
